What You Need to Know
Physicians can enroll and update their information using either:
- The Provider Enrollment, Chain and Ownership System (PECOS), or
- The revised paper CMS-855I Application
Nothing has changed for those using PECOS to enroll or update information. For those using a paper application, as of November 1, 2023, the revised Paper CMS-855I form must be used going forward.
The new form combines the CMS-855I and CMS-855R paper applications and discontinues the CMS-855R.
The current and revised versions of the form will be accepted through October 31, 2023. Beginning November 1, 2023, only the revised form will be accepted by Medicare Administrative Contractors.
